US BANK CASH+ Merchant list?

Is there a list of merchants that are guaranteed to qualify within the cash+ categories. I know cabelas.com for example doesn't count as sporting goods, are there any others? How can you even tell if a specific purchase qualified or didn't qualify? Does it say on your statement

It is hard to tell. You may do a small purchase to see whether it codes correctly, before doing a large purchase.

US Bank does not publish a list of all qualifying merchants, but they should be by MCC code. Example merchants are listed here: https://cashplus.usbank.com/merchants and Cabela's (and Bass Pro Shops) should code as Sporting Goods Stores as they are listed. If a purchase did not trigger 5x, I would contact them since it is clearly in the merchants list and they would credit that unless it was made using a third party like PayPal. One merchant I can think of that doesn't seem to work off the top of my head is Verizon FIOS - I have not had luck with it triggering either TV, Internet & Streaming Services or Cell Phone Providers (I tried the latter because Verizon FIOS does for some reason seem to trigger credits for Verizon Wireless with Amex).

They seem to have redone the Rewards page very recently and I can't find a listing of earnings anymore, which was never easy to find anyway. The earnings are summarized at the top of the second page of your statement, however, although they are not clearly listed next to each purchase as other card issuers do.

US Bank has a feature to export transaction lists to Excel. In that download file, the MCC code is included. There are PDF's out on the interweb to say what those codes mean.
Armed with the transactions for a statement period, and the cash back categories you earned, one can reverse trace how those rewards were calculated, and see whether a merchant acted as you expected.

Verizon seems to be coded here as utility here. It seems that I should review all prior codes

On Max Cash, Verizon FioS does count as Internet, so somewhat surprising cash+ is different.

I prefer that both cell phone and internet would get coded as utility or they get coded in the same category. In this case, it would be easier to use Cash+. For the moment, it is needed to have 2-3 Cash+/Elan cards to cover multiple bills.
