    <description>&lt;P&gt;The FTC also publishes a general identity theft affidavit, which can be obtained from their web page.&lt;/P&gt;&lt;P&gt;The FTC affidavit is generally accepted by the CRAs as satisfying the identity theft process requirment of FCRA 605B that the consumer, in addition to&amp;nbsp; a police report, also include a specific identification of the asserted identity theft, and a specific statement that the consumer never authorized the contested account or transaction.&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;If you are pursuing any identity theft issues with the CRAs, such as a block of the information per FCRA 605B, I would recommend use of the standard FTC identity theft affidavit rather than an IRS version, as the CRAs clearly accept the FTC version.&lt;/P&gt;&lt;P&gt;One of the CRAs is known for actually requiring a copy of the FTC version of the identity theft affidavit as their standard procedure, so I would use that version.&lt;/P&gt;</description>

      <description>What I liked about the IRS form is there was a box you could check if one has no recent knowledge of identity theft but had been alerted that their info may have been unlawfully accessed. Also you could write about how you came across the hack.</description>
