I recently pulled my TransUnion report, and in addition to it having two phone numbers that are fairly associated with me, there is an incorrect phone number there that I don't recognize. As far as I can tell, there is not much other seriously wrong data (other than some reporting anomalies) or evidence of identity theft.

Should I be worried about identity theft? I'm just confused about the chain of events that would lead to a creditor of mine reporting an incorrect phone number since I've always used the same number for many years now, and have not opened any new accounts recently. Does anyone have any explanation for this? The number ties back to an area near where I live. I guess I could pull my other two reports to double check for fraudulent accounts, but I usually try to space out my free annual reports evenly.

Would disputing this mistake cause any damage? (Inquiry that hurts score, etc.)

Nah, simple dispute will get it taken care of with no impact to you. Probably wasn't identity theft, sometimes lenders report erroroneus information: I had one address on EQ that was so laughably broken that it should never have passed any sort of even algorithmic sanity check for example.

Doesn't matter much but some background checks pull the credit report and use the information found on it to lead their investigation: it's worth keeping that data as accurate as possible as a result.
