Work comp bill on my credit report

I have a medical bill showing up on my credit report that is from a work comp claim. How do I go about getting this removed?

Do you have any idea what this is from? Hospital or doctor's bill? How much is it? How old is it (when was the treatment date)? Was the treatment approved by your work comp insurance?

it is am ambulance bill from an auto accident i was in during work, it is a work comp bill, the only one that has showed up on my cr is this one

When did the accident happen? Why didn't the insurance pay for this?
