topic Account duplicates? (Sallie Mae and Flagstar Bank/mortgage) in Rebuilding Your Credit

Hi there! New poster, been lurking for a while. My question: on my reports at all three bureaus, I seem to have two accounts with the same acct number under Sallie Mae, one reporting as 'pays as agreed' and the other reporting as 'debt included in/discharged in BK.' I have a four-year old Ch. 13 on file (thanks, housing market!), but of course my student loans were not discharged. Since I'm still paying that bill, and it has not been reduced at all in BK, I'm not sure why it's showing up on my report as 'included in bankruptcy' with an ugly little orange flag/reporting a collection as the worst action on record (I have a few delinquent payments in the past, but the account is current.)

Sallie Mae Account 1 reads:

Status as of: Apr, 2014
Date opened: Feb, 2006
Date of last activity: Not Available
Loan type: Education Loan
Terms: 300 Months
Description: None

Sallie Mae Account #2 reads:

Status as of: Nov, 2011
Date opened: Feb, 2006
Date of last activity: Not Available
Account number [Exact same digits as above]
Terms: 300 Months
Descriptions: Debt Being Paid Through Insurance

Second question: I also seem to have duplicate accounts with my former mortgage holder, Flagstar Bank. I only ever had one mortgage, and in order to get the company to agree to a shortsale so I could move, I had to file bankruptcy in the end. Each bureau is reporting two accounts, however, for my one mortgage with them:

Flagstar Account #1 reads:

Status as of: Feb, 2012
Date opened: Sep, 2007
Date of last activity: Not Available
Loan type: Conventional Real Estate Loan, Including Purchase Money First
Terms: 360 Months
Account Type: Installment
Account holder: Individual
Descriptions: Account Paid In Full For Less Than The Full Balance (p.s., this one is also reporting a 90 day late payment in Feb. 2012, three months after the bankruptcy in which it was included was dischared...)

Flagstar Account #2 reads:

Status as of: Nov, 2011
Date opened: Sep, 2007
Date of last activity: Not Available
Account number: Different account number from above
Terms: 360 Months
Account Type: Installment
Account holder: Terminated
Industry Savings And Loans - Mortgage
Descriptions [?] Account Paid In Full For Less Than The Full Balance (this one shows all of the delinquent payments I made in order to try and get them to agree to a shortsale. Flagstar Account #1 does not have these details).

Any insight would be greatly appreciated. I'm wondering if these are in fact duplicate accounts and if so, would the best action be to contact the companies and ask that they be removed, or to just dispute it with the bureaus--and if I dispute it, what exactly is the appropriate category for the dispute? Thanks!

-G
