It doesn't say with or without prejudice, so how can I tell? Apparently the trustee never received our checkstubs.

I would definitely call your attorney to find out. Sorry they let this slip through the cracks, I'd be livid.

Have you looked at the Docket Report on Pacer? About 2 weeks out from discharge, I receved a letter from the Court Clerk and also my attorney reminding me to do the exit financial course and it's listed in the report with copies of it.  I realize it's not the same thing, but surely someone, somewhere should have pointed out to you that they still hadn't received that.  I hope you can get it reopened and corrected asap.
