ID is ID no matter what state it's from. It should also not matter what state you incorporate in. The bank will most likely want a copy of the IRS EIN confirmation, but otherwise you should be good.

A real address is always going to look better. Some banks will not accept virtual addresses/P.O. Boxes. I will assume yours is a home based business? If so, it will still look better to a bank, especially with all the fraud going on these days...
Some banks will turn you down, actually. US Bank, for example, will require your LLC to be registered as a foreign LLC in IL for them to open an account for you in IL. Since Indiana has US Bank branches too, you can try opening your account there. I spent many a month from 2017 to 2018 tackling a similar issue.
You could try one of the new online business checking accounts with no fees and totally online application. I went with Novo Bank.
