Let's say that someone pays it off in 13 months instead of the 12 month 0% offer.  Is there any retroactive interest charged, or do you only have to pay interest on that last month?

That only applies to store card 0 percent offers as a rule.  With a major your interest starts on the balance at the time the interest kicks in.
