I've used the companion certificate in first class twice now. No hassle... book online... easy. You can use it for a first class or economy class ticket.
The only thing to know is that they limit it based on fare class. For First class there must be seats available in A or I (basically the cheapest fair classes) and in economy in L, U, T, X, or V (again, lower end fare classes). If you're booking 2-3 weeks in advance there should be zero problem getting tickets in those fare classes. Also the primary ticket will earn miles but the companion ticket does not.
The other thing is it gives you access to Delta skyclubs (when flying on delta). The cardholder gets in for free and you can bring a guest for $29. Alternatively, you can get DW a reserve card as an authorized user (for $175 a year) and then DW can get into the skyclubs any time they are flying on delta (with or with you) for free.

Beginning April 6, 2017, Medallion Members traveling with one companion will be eligible for Complimentary Upgrades to both First Class and Delta Comfort+®based on the status of the higher-tiered Member, as long as the companion is a SkyMiles Member, Medallion Member or partner airline elite member, traveling in the same reservation.

Have to clarify. Language matters.
There is a $25,000 MQD waiver which is part of the Delta SkyMiles program. You can have the Delta Blue, Delta Gold, Delta Platinum, and Delta Reserve in any combination and all spend on all these cards will apply toward the one MQD waiver. There is only one $25,000 MQD waiver, then that is done. ( If you want the Diamond Medallion waiver, get to $250,000 spend on any combination of those Delta cards ).
For each card Delta Platinum or Delta Reserve, within AMEX, there is a MQM Bonus and SkyMiles Bonus. You spend $25,000 on the DP, $30,000 on the DR to get the first MQM and SM Bonus, then another $25k or $30K to get a second MQM and SM Bonus. Those bonuses do not transfer between the cards, but you can earn 4 across two cards.
MQD = Medallion Qualification Dollars
MQM = Medallion Qualification Miles
