There were two different cards:

5x Thank You Preferred:  Up till April or so, people (including me) could apply online, using a link dating back to April 2011, to get the 5x benefit.  This was eventually shut down (and another one had been shut down earlier).   Like you, I have heard no reports of anyone since getting a link to this offer.

5x Thank You.   This was available only in branches.  It was there in May, I plan to call tomorrow to see if it is still available.
