I would say keep using the US Bank sp cli online tool until it reaches $5000, and then call and request the cash+. I went through this conversion process around one month after I got the cash rewards card in the mail. The conversion is based on some sort of automated system relying on internal scoring. They approved me instantly for the conversion although I had done exactly one transaction on the cash rewards. But the csr lady did say that was a bit unusual, usually they like to see wait six months before they do the conversion.
As for the fido amex, I would say wait six months. If they still don't approve, probably you will have to wait until the baddies fall off.
