I recently moved and put several large charges on both my Amex Gold and Magnet--including nearly $6,000 to Atlas Van Lines. I hopped on the Amex chat feature a week or so before I moved to alert them that several large charges would be coming through. They thanked me for the heads up and I did not have any issues.
Check your BCP offers. I looked earlier today and have one that gets you $10 on individual transaction of $1000 or more. Can use the offer 3 times. Maybe you can get the hospital to work with you on dividing that $4500 into 3 pieces. $30 is $30.

I paid a hospital 3400 with my everyday Amex card. The card was relatively new at the time and had a balance of 4800, and a CL of 25K. I didn't call them and ran the charge through and was approved. Your credit line in most cases is the amount you can charge without asking for permission. If they need to verify, they will get in touch with you. They have advanced fraud detection now which will result in fewer contact verifications.
BTW, before you pay, ask the hospital if they will give you a discount for paying in full, AND send you a statement that you are paid in full and will receive no more charges for the services in the future. I did, and they gave me a 10% discount. Much better than the reward dollars you'll still get on the charged amount. It's like cashing in on a Amex offer. That was $340 off my bill. And I never got those dreaded bills for "other services" months later.
