Hey Guys,
Was wondering if any of you have sent this form in? I submitted it to the IRS and put amex as the person to receive it however I recieved a copy of it myself? Does anyone know if its customary for the IRS to send both you and the authorized party(being AMEX) a copy, or do you think they made an error and just sent it to me?

It was most likely in error. The IRS should send the transcript to the address located on Line 5 of the form only. I have heard of individuals receiving a copy in addition to the Line 5 recipient, but more than likely it was a mistake, or the form was completed incorrectly.

Yes, when I submitted (originally) the form 4506-t, I did get a copy of the transcript myself in the mail. Give it 5 days, then call Amex to see if they got it, if they haven't gotten it, go ahead and submit another request.

Sorry to bring this folder up again, but AMEX needs this from me. The thing is, I don't know what, who and where to get this form from, let alone filing it, lol. Can someone send me some information as to how do I get this done?

You can download the form from the internet. I've done it before.
If the IRS already sent you a copy of your transcript and AMEX hasnt gotten it, just call the rep at AMEX who is handling your FR and find out where to send it to. It happened to me and I sent them (AMEX) the transcript directly after them not getting it for 2 weeks. Once they got it, the next day the FR was over and accounts cleared. I believe that you can also request a copy of your ranscript electronically. There is a 3rd party service thatll speed the process up. http://www.veri-tax.com/FAQs-4506T.aspx is the link I believe.
