Well, number 2 was increment redemptions (e.g. multiples of $25). But since both Amex and USAlliance have stopped dpoing this, it might be drifting down. And, going back a bit, #1 is people who call in requesting unneeded CLIs all the time (because it is there), causing unnecessary costs for the issuer, which then impacts everyone. God, I still get angry when I think about that!
FWIW, doing an Amazon UK reload works fine. So there is potentially a "simple" workaround! Reload then use account to pay. Thank goodness for Fintech!

Slight correction according to sources that wish to stay anonymous, the person in question never called for CLI, person was hitting the button till that faithful day when person in question had to call regarding unrelated issue (they read online that CSR can see requests), so they asked if they can see requests.
The person in question did not ask since.

I spent two trips in the UK using a SoFi card this October and November (since AMEX cards are often ☠️ there, this became the "non AMEX default"). Boots and local stores didn't have problems. ¯\_(ツ)_/¯
