I got rid of the card months or years ago, since I had no intention of using it (i.e. not carrying it around, though using it on the insurance company site), but I saw no reason to close the account.

That's a very good point. When I have negative balances on cards, I definitely *do* receive a statement. With $0 balances, it varies by bank--sometimes I get a statement, sometimes I don't. But, yes, you should have, especially given how long it's been.
Did you check your payment method info on your insurance company's website/app? Have you checked your credit reports, like on Credit Karma? (If I've missed any of that, sorry.)

That's precisely the problem! The payment method on the insurance co. website shows the last 4 digits of the credit card! But nothing more, and the insurer cannot tell me who the payee is.

If you are trying to find out what credit card is issued just look t what cards are on your report. You should be able to discern hat card you decided not to use . 2 years is not a ong time credit wise so it should be on your credit report. If you know how many credit cards you have right now in your possession, just go down the list of open credit cards until you see the one you no longer have.

If i was paying an insurance company for a policy i would DEMAND the info and go up the ladder if necessary.

Unfortunately, the insurer (or company) isn't really required to furnish that information. The insurer doesn't own the payment data. If the OP used it in the past, then that's the payment method that was either stored or used at some point, but all the company can do is either remove it, edit it or add a new method. Depending on the UI or system constraints, they're really not responsible to release or know which lender it is/was nor is required to release the account number. If their system only shows the last few digits of a payment method and doesn't have any other identifiers other than V/MC/D/AX, that's up to the policyholder to reconcile it in this case.

Playing devils advocate here, have you ever had an insurance policy that you paid every several years? This story is definitely perplexing. I have several insurance policies for different reasons. One pulls the monthly premium from my checking account. One i pay quarterly, and the last i pay twice a year.
I never saw an insurance policy of any kind that can suddenly pop up out of years of dormancy to charge a credit card or debit a card.
