What happens if an online app isn't instantly approved?

What kinds of extra questions and documentation requests might you receive in the mail if an online application isn't instantly approved? I'm not applying to Amex. Credit union Visas, most likely.

1. Income verification - 1-3 last paystabs.

2. Identity verification - photocopy of DL or/and SSC

3. Adress verification - copy of utlity bill.
