I received one of these from MF and the reasons it gave was a physical address from an address I resided at over 5 years ago, what does this mean?

Just means the address got posted somewhere on a site which is used for exchanging (buying / selling) personal information or similar. It's not a huge concern in this case... I occasionally get one for a yahoo mail account I setup in 1997 (and still use admittedly) with an old password, but it's been reposted twice on some random site that's being watched.
Some information presumably is more important than either your or my cases.
