Would you believe absolutely none? Every time I have used NFCU Amex (for “30,000 reasons”) it has worked flawlessly. The vending machines in the Underground stations even accept Amex for topping off your Oyster Card (tap card for the tube and buses). I would say at least around London it’s at near-parity with US acceptance.
I’ve actually been paying with my phone 90% of the time. ApplePay and other contactless methods are very well-accepted over here and it’s nice to know I’m always getting 4.5% back on USB AR no matter how things code.
They always say they are working on more acceptance world wide.
For me, it's a moot point given Amex's high AF / FTF trade-off.
(Half of my cards have no fee and no FTF, all but one of which also provide cash back. And then there's the CSR, which gets pretty much all of my travel spend. But then, my mileage varies.)

So it's okay to have a high AF with no FTF as long as it's the CSR because of acceptance I'm assuming?
Personally, there are more transfer partners to like with AMEX but as you mentioned mileage with everyone varies. As long as you find redemptions you like then it's all good.
