I need this as well, I was able to pay AT&T directly through their phone system for $69 (with confirmation letter) after the rep told me no, so AT&T technically has my money now! 
I'm dealing with Diversified Consultants who already said they wouldn't do any kind of deletion for any reason even though I never got a last bill (the rep when I closed my one month old account said it was zero'ed out because it was so new when I switched to verizon). I asked them when they sent a notice, they said April 9th, so I asked them why they were reporting on something that I had not had time to legally send a letter back for DV - they told me they can't answer credit questions...hah.
