Old account sold to new CA

I am looking to buy a house in about six months through a VA loan.  I have an unpaid medical bill in the amount of $688.  It was sold about two months ago, not it appears as a new collection.  I want it deleted from my report, but I am unable to PIF all at once.  I spoke to the CA and they said I could make payments and they would delete it.  I also spoke to a lender, and they said that it could be overlooked since it is a medical debt.  Should I go ahead and pay it?

I would get the agreement in writing first from the CA, before paying.
