I sent a dispute letter to a collection agency who is reporting an ambulance transport for my daughter who is underage. what i got back was a patient care record with my daughters name misspelled, the wrong date of birth, no social security number no next of kin info and the mailing address is listed as unknown. none of my information (name, ssn, etc.) is listed anywhere on what they sent me nor is there a signed agreement that says that i agreeed to pay them to transport her to this hospital. since this is not adequate validation to me what should i send to the CRA? help with wording would be great.
