"Serious Delinquency" but perfect credit reports?

Hi, my husband and I are working on getting a new mortgage right now.  We looked up our free FICO score that we get with Discover, and we also got a report from Fifth Third Bank disclosing our credit score, which they used in the mortgage decision process.  Basically both of those reports give us the score, then the "Key Factors" under which is just says "Serious Delinquency".  So we pulled all three of our reports from annualcreditreport.com and all of them are totally perfect, with no negative information whatsoever.  After calling all parties involved (Fifth Third, Discover, individual credit bureaus) they all say they see that out credit reports are perfect and basically have no idea why anything says "serious delinquency".  We have no idea what to do to figure out where that is coming from, why, and how to fix it.  Any insights?
