Here is a sample letter I faxed ChexSystems to get my listing cleaned up. I attached discharge paperwork and used faxzero to send the fax to them for free. It was removed shortly thereafter, within a week or less. (Took time to get the new Chexsystems report in the mail.)

My bank has informed me that there is negative information reported by Big Bank included in the file Chexsystems maintains under SSN: 555-55-5555 / Consumer ID: 111111111. Upon ordering a copy of the report, I see an entries for various banks I have had in the past. (Bank of Big America, US Big Bank Corporation, Big Wells Fargo, Big Wachovia Bank.)
I filed for bankruptcy chapter 7 on April 3rd 2014, and had it discharged on July 8th 2014.
Please validate this information with the attached bankruptcy discharge papers. I am requesting the removal of these items per your policy, please. Would you also please provide me an updated copy of my Chexsystems report showing removal of said items. Thank you!
