A couple years back and they actually locked my cards and made me fill it out but each time I sent it I did something wrong and you only have 3 tries before they close your accounts. I recently applied for the Gold Card and it went pending and what comes in the mail a 4506-T Form. I will fill it out correctly and hope they approve me this time. EX score was a 745 at the time of the app and UTL is 5%. Will update once I send form in. Does anyone know how long it takes after you send the form in?

Last time I did it they took 7-10 days to fully submit to approve the request.
Just go to the IRS website and download the form.... https://www.irs.gov/forms-pubs/about-form-4506-t
If you don't have a PDF program to E-Sign with then just print it, sign it, scan it, and upload it.

I saw that I could download it but I wasnt sure what to fill in and I want to do right the 1st time around.
Name, Address, 2 years of tax info, check the box, sign/date.....

I came home and tried uploading it through the website and it said there are no further actions so I faxed it to them I will call on monday and make sure they have received my 4506-T
Follow up with them is key.... they tend to be a bit scatter brained in that dept. It's supposed to be completely automated though once they receive it, review it, and forward it. Even when you do it online they will give the excuse of 72 hours to get it into the system before it shows up but, once it shows up and they send it in to the IRS they have a response back quickly and then approve/deny within a day or two.
