** 3/1/22 UPDATE **
Card finally arrived in the mail and I was shocked to only receive a $500 limit. Prequalifier said $14,500. I was beyond disappointed. Called to find out why the difference, and they gave me a canned response about it's only an estimate, blah blah blah. I sent w2 showing 130k income, drivers license and SS card because they said they couldn't verify identity. DTI is 2%. Experian score was 691. Only lates were a few student loan lates from 2018/2019. I had Navy cards at 20k, 1500k, Amex 2k, Macys 2900k and Discover 1k at the time. Recently (after FNBO inital approval) got Navy increased to 26k, 4500k, (2) 3500 PenFed, 2k Apple card and Macys increased to 8900.
I decided to just cancel the card. I know I already got the hard pull, but I dont think the 1 inquiry with the other 3 new ones affects my score. I'm hoping it won't even show up on my report.
**UPDATE:
I called after receiving the letter below and they told me they were mailing me another letter that tells what was needed for further review. The letter asked for last 2 years w2s, picture ID and signed SS card. It asked me to mail it, but I called and asked for an email/fax number. I emailed the docs about 8:30 am, and received my approval email around 6 pm. It didn't list amount, so I'm assuming its the 15k they preapproved me for. Will post later once I received the card in 7-10 days. I do think the LexisNexis freeze created the problem.

A couple of weeks ago I did the online pre approval app and it said I would be approved for 15k, but I was denied about 3 days later. I've never seen this reason before. Does it mean something didn't match up with name, etc? I just received letter today so will have to wait until Monday to talk to someone at FNBO.

Hi @TC1212 you can call FNBO and find out what kind of verification they need from you. You should be able to recon FNBO. Call them.

Are any of your other reports frozen? LexisNexis, SageStream, etc?
@FinStar @Yes, Sagestream/NexisLexis is. I didn't realize that mattered.
