Have an old inq on my TU report shows it was made on "December 06, 2014", due to fall off in...January !
I thought they fell off 2 yrs from "the date of..." !
Am I wrong ?

Shor answer is your correct - 25 months. However in practice I have experienced 19-24 months.
As always, YMMV....but for me this year I have had 3 inquiries fall off all 3 CRA the month in which they occurred 2 yrs prior.
HP reported INQ's that are past 12 months no longer count against your FICO scores at all. (penalty ding)
However lenders can and sometimes still consider those into their credit card decisions

Does the inquiry impact lessen as the 12 months progress or does all of the points come back at the end of the 12 months?

That is profile dependent I do believe.
For mine the impact (FICO points penalty) used to be 6 months and longer but going forward it has reduced back to only 3 months and even less
