You do have to activate the bonus categories every quarter even if you plan to keep the same selections.
Note that many internet service providers fall into the "Cell Phone Providers" category rather than the "TV/Internet/Streaming" category. You may need make a small test payment to see how it codes. If it turns out you made the wrong choice for your first quarter, you can try making another test paypment via Paypal and see if that changes the coding. My ISP is a phone provider when paid directly but a TV/Internet provider when paid via Paypal.
