I applied for 3 CCs last week. 2 pulled EX, and the last one pulled EQ
The last one found out I have 2 recent pull on EX and denied my application.
I'm wondering how this can happen? Should the one who pull EX have visibility how many inquries i have on EX?
Even my EQ Score Watch notifyied me about the 3 inquries. Anyone knows how the inquries are reported?

Sounds like the one that pulled EQ also pulled EX. Many companies check multiple reports. In a recent Amex denial, they actually pulled all 3 of mine. Capital One always pulls all 3.

Have you checked your CR since then for inq? It sounds like the last one must have pulled EX in addition to EQ. I can't think of any way that they would know what's on your EX report otherwise. Which CC is it? Some do multiple pulls. Amex usually only pulls EX, but sometimes also pulls EQ and/or TU if someone is on the borderline of approval. It could be a situation like that.
