Do you know who Chase pulled, and if they pulled another bureau the 2nd time? That might explain the difference in your 1st and 2nd attempts. Amex virtually always pulls EX, so it has to be something with your EX report that Amex doesn't like.
Its also possible AMEX pulled more than EX, seen the low EQ score.............

Are you using Firefox as a web browser?? If so, use IE. Its a known problem for amex decline letters not showing up on Firefox.
