This stinks of a management/culture issue. Internal audits with progress reports and in house documentation should have resolved many of these nonconformances.
Flies on food and buzzing around are disgusting. A film (likely mold) on surfaces where ice is held is unacceptable. The surface should have been swabbed by the inspectors. These lounges should close nightly, have a well documented after hours cleaning program and a preopen inspection by a different crew team.
