I would never dispute an HP with any creditor that I have a relationship with. It can create havoc on your credit reports (fraud alerts, etc) and can make you delete a positive tradeline forever.
I got an HP from a Comenity CLI request from the Robolady last month. Literally came as I was on the phone with her. Very annoying.
Did you recently shop with Kohl's and make a big purchase?
Regarding the comenity account you requested an increase to your CLI(whether it was approved or declined) so they have a right to pull your credit(whether you requested it to early or not which might seem wrong if they have x rule in place you can't get a CLI before x date of having card) sometimes it is a SP, but whenever you ask for a CLI expect a HP unless it expressively says it will be a softpull or similar verbage. So you are out of luck there on the one.
Have no clue w/regard to Kohl's someone that has the card might be able to give more insight on that, but Cap1 services the Kohl's account so might want to give the credit dept. a call and have them explain that if you didn't request anything as you mentioned.

That's because by law they don't need any reason to pull an HP as they're already your creditor and already have permissible purpose. If they wanted to pull an HP daily, they're welcome to.

I guarantee you that both HPs were allowed and legal under PP and fighting them with the CRAs will possibly lead to havoc.

HPs from CLIs and applications can take weeks to appear. This isn't unusual. If you applied for a CLI in May, it can pop up weeks later.
Always expect an HP from a CLI, celebrate when it's just an SP.
I feel like I've been reading more and more people are getting HP's with Comenity when asking for a CLI. Not sure about Kohl's, not sure if you said you requested a CLI wiht them as well or not?

The truth is any creditor can and will run an HP even if they promise not to if you have borderline credit. They'll do the HP just to see if they can find any reason to give you a CLI. The more borderline you are, the more likely an HP is. Stop asking for CLIs if you don't want HPs.

That's up to you. It's just one HP, I wouldn't bother, but if it means that much to you, go ahead and write them. Chances are they will just say "You requested a CLI, we did an HP which we are allowed to do." I have never disputed inquiries because inquiries don't matter at all unless you have a poor credit profile (which I do), and in that case, it's best not to do anything that will incur a possible HP. Until my scores are in the 740+ range, I don't request CLIs because I am a more likely candidate for HPs.

Which creditor above is not your creditor? If you apply for credit, they are allowed to pull an HP. If you never applied for credit, then they are not allowed to pull an HP.
