I've seen that DV and I love it.  It's perfect.
 
Remember to print your name... or just type it... do not give them your signature. 
 
That's great... just put your own info in and you are ready to send.  Good work!
 
Please though go to your DV post, click the 'options' menu on the right side, and then edit.  Take out the first paragraph with link that you copied and pasted regarding debt before a MOD comes along and spanks you for TOS.

edit: Oh on your actual DV omit that paragraph of course, as well as the very last line unless you have that item to include.  I don't think sending anything is necessary IMHO.
