Is a change of address form good enough to verify?

I dipsuted some things with Chase and they sent me a copy of a change of address form back in 2002.  Is that all that is needed to verify a debt?

What do you mean by "disputed things with Chase"? 
 
Did you dispute via the CRA?  Through a CA?
 
Did you send Chase a DV?
 
I don't think a change of address form qualifies, however if you sent them a DV, they do not have to comply, only CA do, not OC.
 
If you disputed via the CRA and they verified, you can send an investigation letter to the OC.
