Is a collection account from 02/2003 still affecting my score?

I bought my fico scores (from myfico.com)  and on the listing of "What's hurting your score" the number 1 item is "you have a collection account...."  This collection account is from 02/2003 and I just paid it off.   Does Fico list these items randomly or in order of how bad they are hurting your score with the first item listed being the main thing that it hurting your score? 
 
The #2 item is "you have made heavy use of your available revolving credit".

Fico nearly always finds something to complain about!
 
#1 My only baddie was a 6.5 year old collection.  When it dropped off (age) my score went up 50 points.  Others have had very different results.
 
#2 - High util is a score killer.  It worth 35% of your score
The slide from grace is really more like gliding
