I really appreciate the help you guys are providing.  I will call NACA tonight when I get home - I am not allowed personal calls at work. 
 
I added this paragraph to the letter, right above where I was putting in my address at the end... is it a good idea or should I just leave it out?
 

In response to the letter I received from your office on October 12, 2007 (dated September 11, 2007 and addressed incorrectly), I acknowledge that I did in fact fill out an application at Rooms to Go in Knoxville, Tennessee.  However, I did not receive an approval or credit card from Rooms to Go, nor did I purchase any furniture or merchandise from that vendor.  I certainly have never purchased any refrigerators as indicated on the statement I recently received from “Rooms to Go/Retail Services” dated December 9, 2007.  The statement indicates that two charges for refrigerators originated in October 2003, yet on my credit reports, this account indicates that it originated in 1999.  Therefore, any and all charges to this account are fraudulent and/or the records have been falsified.  As such, I demand that a full investigation be made into this account and that I be provided with the results of said investigation.  Failure to do so will leave me with no alternative but to pursue and any all legal remedies available to me including, but not limited to, willful noncompliance, slander of credit and defamation.

jen I would not add that. 
 
Though it's honest, it will only help their case not yours.  It's best to not do any of their job for them... they may not be able to find a signed document and if they don't... that written acknowledgement will hold up against you legally.

they already have the signed document - they sent me a copy of the application that i filled out.  with the application, however, they also sent the creditors rights or whatever that comes w/ the credit approval that you have to sign, and it was not signed.  it was blank.

FYI - Verizon blew me off saying it was a DV - then another office called saying they would investigate (still haven't heard from them again) -- next step is atate AG and FTC for me if the 30 passes and I have nothing.
  I would tweak as follows:
Should you not respond with the investigation results within the timeframe as required per the FCRA, I will assume you have no documentation of this matter and are therefore negligent in providing the credit bureaus with the information.

No - this is  an assertion of your right to have the account investigated.  Let them produce what they can (if they can) - you are NOT required to provide any info, they are required to provide all they have.
