On a few it has comments along the lines of "this creditor reports to a credit agency " , " this creditor reports above x dollars " etc.
Is this commevt requirement for medical collections ?

Many states now require a notice that they intend to report to the bureau(s). There is a myth that medical debts cannot be reported as it is a HIPAA violation and that is just it; it is a myth. As long as they don't reveal actual details in the trade line it is not a HIPAA violation to report the debt. Due to changes in collection laws they cannot report medical debt for 180 days past the date of service/care to give insurance time to process claims and appeals as well as for the patient to pay their portion. Once 180 days passes they can report the debt as in collections.
