is this a letter i can send to them each one:

 

Mod Cut! this site has a simple template where i just put the information in and it creates it for me. should i just use that one or are there betters ones for removing the hits on the report?

Yes this can work but I suggest tailoring it to your unique situation then it seems more doable.
