HIPPA Letter Process - Step 3 Questions

I'm preparing the Pre-HIPPA Medical Dispute Letter for mailing to EQ and EX (the medical debts are not on my TU report).

Looking at the CR I got directly from EQ.  I can't find a report number on the crazy thing anywhere.  It just says "Report pulled on 4/10/2009".  So do I just indicate that in the HIPPA letter to them?

I got a report number off the EX report.

Yes, I pulled directly frorm EQ.  I found my order number in my order history.  Is that the same as the report number?
