I have read and re-read the HIPAA sticky to avoid asking a question that has already been answered..to no avail. Here's the question: I have a medical collection that is PAID reporting as UNPAID. I did the pre-HIPAA dispute letter to Equifax. They came back "verified" "Equifax verified that this account belongs to you." I had the HIPPA letter to the OC prepared, then received the dispute results from EQ. I'm not sure which letter to use because there are two issues here. Do I just use #3- Alternative DV Letter to CA?

One collection is reporting only to EQ. The tricky thing about it is that it was "paid" by taking my state tax refund. So I never received anything from the CA. They probably don't even know it's paid, as the payment went straight to the OC.
The other one is for $279 and is reporting to all 3 CRAs.

I would think that the CA knows that it was paid because the OC had to pay them. Do not tell them anything! Just mail the DV as it reads.
