A 689, but why?

Recently trying to get a refinance I learned that my Fico score had dropped down to 689.  After reveiwing my credit report I learned that I had a dispute 5 years ago for $45 that was eventually paid and no late payments on 4 credit cards 3 of which are paid off and 2 of which are over 6 years old. But the report saud that two of the cards are new which is suspicous (2 years old) and the dispute 5 years ago.

 

 

Does anyone know how I can raise my score. I felt kind of betrayed by this score since I try to never be late and keep my balances down.
