How do I interpet this information in my credit report (see details)

The following questions pertain to an account with a "Closed" status, a "charge-off" payment status, and revolving term.  It was sent to collections, so it also shows as a public record judgement.

 

  1. Why is every month being reported FP "failed to pay" if the account is declared closed and charged off?
  2. In the payment history section, data is included for 2010, 2013-2017, but not indicates "data unavailable" for 2011 & 2012. Why the 2 year gap?
  3. Is the balance in this account calculated into my utilisation?

I can include a picture if someone provides instructions. Copy and paste didn't work.

Charged off doesn't mean you don't have to pay.  It really means nothing, but that you haven't paid. If you want it to stop reporting every month as unpaid, you have to pay it.
