how will unauthorized inquiry and cc approval/ closure affect my score?

Hi folks-

I'm a newbie but have been reviewing many posts here. Just for background: My husband and I are looking to improve our credit scores to do a refinance. 6/19/09 I had EQ 698 TU 715 EX 641 (lending inquiry). When we met with an agent 10/1/09, without pulling my report she informed me that EQ dropped to 671. We know we have super high Util ratios right now and are crossing our fingers to do some serious payoff in about a month. 

 

But aside from that, I'm wondering about another activity on my account and what affect it has on my report: I recently applied for a credit card for my business. The advertising from US Bank suggested it was a business card, but it turns out it was a personal card. So they botched things on several levels: they made an inquiry to my personal account, then approved me for the card, actually issued the card but on the same day someone reviewed the account and changed their mind and closed it. So now I have on my personal account a line for an additional inquiry on my personal account that I didn't authorize, plus a new account listing, plus an indication that the account was closed right away. I have contacted the Comptroller of Treasury and filed a complaint for fraud and complained directly to the company.

 

My questions are, should I dispute this, and if so, is there a way to request US Bank to remove the inquiry or do I need to do this through the reporting agencies? Any recommendations on the best way to do this/ is there a specific term I should be using? Will the dispute on my record hurt me more in terms of my score than the false activity? (I ask this because the LO said Experian listed #8 "too many inquiries in the past 12 months" - even though there have only been 2...but I've seen some postings where folks' scores dropped after filing disputes or after the dispute was resolved, even if in their favor)
