https://online.americanexpress.com/ecomm/esign/en_US/verifyIncome - if you apply for a CLI and get 4506'd and didn't fill it out at the time of the request
Also, if you don't want to wait on the mail to get your reference number to look it up give these guys a call and they'll e-mail it to you.
