Prequalified for Amex Everyday but they asked for the 4605 form which was not sent to them so they cancelled the app. How long should one wait to reapply and will they continue to ask for the form? Thin file no derogs....

Don't think anyone can answer that other than to say that they probably will keep asking for it if your file appears the same the next time you apply. For me I would just say the hell with Amex and go to another lender then maybe check back in a year once you have a thicker credit file. Up to you though. I have plenty of cards so won't put up with that from a lender, but for somebody without much credit history they may feel differently.
