Well, in my case someone forgot to tell Amex that as I have well over a 25K line on one of my Amex cards and have yet to be asked for the first financial document.
Well the $25K is just a rumour and amex has the right to ask for FR at any given time, even if the limit is only $5K. Most people think the FR starts around $25K but there is nothing set in stone about it. I have seen people with $30/$40K on one card alone and never been asked for FR. So i guess for the FR it all depends on each individuals accounts, credit profile, and overall exposure with them.
