Denied credit limit increase....should I be worried?

Late 20s (Ok, I turn 30 in a month Smiley Sad), reqeusted a credit line increase with my CITI card and was denied because according to them I requested one on Dec 14 (which I never did).  When I told the rep I never requested an increase he said "maybe you did it online" in a very suspicious tone....

The credit line on the card was $12K when I got it when I was 19 (the good ole days), I think they increased it to $15K a year and half ago (didn't ask for it at that time either), supposidely it's at $18K now.

Fico is 800-810, credit from credit karma is around 830.  Never any late payments or unpaid bills.  On my credit report the two things affectings is lack of other types of credit (don't need a car/house loan so that'll just be) and age of credit history (I did open an AX account two years ago, only account I've opened in the past 10 years - more on that later).  But the age of credit history is removed a month ago and "revolving balances are too high" is mentioned.  I do run the CITI card up $5k-$12K every month, pay it off, but I guess since the credit line on the card is $18K it seems high to them?

I have about 7 cards (don't ask - college days, where you get a free box of pizza for signing up for a card LOL).  CITI card is my main go to ($5k-$12k a month), BOA I rack up $500 per month (5K credit line), AX card is about $1K a month (15K credit line) and the others I barely use (1-5K credit lines)...Random small purchases every few months (Coffee and what not) just to keep alive.

CITI said they don't do a hardpull to credit line increases...do you think they signed me up for some auto credit line increase?  Should I look into why they said I requested a credit line increase two weeks ago when I didn't?  Obviously if I knew this I would not have requested one now.

Also own my home and my cars so I don't need a loan.  My cell phone is on my parents family plan even though I pay for it...should I move this in my name?  Or will paying a cell phone bill every month not really improve my credit?  Also pay for my family's car insurances, not sure if I can move that to my name or if that will even improve my credit score if I do so.

You could always add a secured loan to show some credit mix. Citi will do HPs on request. They also do SP CLIs.
You can check your reports for a HP.
Every 181 days you can request a SP increase after the first increase.

I don't see a problem with your spending or your overall card situation. But I see a big problem with the fact that the rep said you requested an increase last month. I'd investigate that further. You may want to pull your full reports directly from the bureaus or annualcreditreport.com as those will show any soft pulls. And you should call Citi for more detail.
