You apply for each FI's card (design) individually. When you apply for the affinity cards (eg - Intrust) you will be selecting the "design" when you click on the card and proceed with application.

Oh I know. I mean if you check the list there are a couple banks that let you pick a design, multiple choice.

Oh, really? I haven't come across any of those in the hundreds I've looked at. Which Elan supported FI's offer design choices to "choose from"? Thanks!

Well according to the list above, Associated Bank has 3 options and Intrust Bank has 14 options.

Those are the affinity versions I was speaking of. You would go to the FI's website, select the card you want to apply for and click on the "apply" link. You will then be applying for your selected affinity organization card, or rather...your "choice design". There is no multiple choice for design on any one single card's app page.
Note that many FI's have subtle differences in card design between the different card type/level. But you still can't choose the design...you select the particular type/level card and get whatever design it has.
