This is approximately what I would do, if the timeliness of braces were important, and given the facts at hand. Geeting both UTL under 89% would be of some marginal benefit. But again, at your scores, Finances befor FICO. That is a debt load that needs to be addressed. The answer is not more debt.

You already have Amex, so if you have preapproval, you can try, but I would not with maxed cards
Your limit on the existing Amex would not cover the braces, and I really do not see them going with higher SL while you have two cards at 90% utilization.

OP, here's my suggestion. Since you have $3,500 in savings, I suggest using that money to start paying down those two maxed out cards. As Remedios said, I do not see them approving a 0% interest card at a SL you will need. And, even if they did, then you would have 3 maxed out cards.
So, I suggest putting the braces on the back burner until you can get both of those cards to less than 28.9% UT.
EDIT: Oh a bonus. Yep, throw that at the card that has the highest interest.

It will because you're taking on additional debt on and you're already struggling with the one you have. The interest on that Capital One card is probably as high as your braces payment will be.
I know braces are important, one of my kids just got hers off last year, but you need to wait a little bit longer till you're in a better position.
The choice of course is yours, but if you risk your current rebuilding efforts, what then? One late payment and it's done.
