My Verizon bill is about $250 per month too. And, for 5 lines with smartphones, it's not bad really. What cracks me up is Sprint - cut your bill in half, but you only have half the coverage of Verizon in my area.
US Bank 5% for cell phones is a good deal and that's a major reason I got this card too.

Just received my welcome e-mail. They never called to confirm anything. Was approved for the Cash+ Visa Signature so I am guessing my limit is at least $5,000 to get the Signature version but doesn't state in the e-mail.

Several reported banks have modified their stance on the $5K CL for a Visa SIgnature. However, to my knowledge US Bank still stands firm that you must have $5K to receive their Visa Signature. So likely you made the cut as the US Bank Cash+ Visa only comes as a Signature. Congrats!!!
Congrats on your approval and welcome to the U.S. Bank Cash + family! Just like others stated, definetly a VIsa Signature if it is a Cash +. If your email has all the Cash+ signs that they love to put in then you are golden. Only thing with U.S. Bank is that they state something along the lines of "if we can't approve you for a Cash + we will send out a regular Cash card".
EDIT: You said you were approved for Cash + so nevermind on that. Congrats again.
I had that issue with them when I started building credit, but with your scores I see no problem
