What do I need to put in for the shopping cart? Do I have to make a purchase

I know you don't have to make a purchase.  You get the card and end the session without completing the purchase.

I don't know if it affects the starting limit by putting a higher total dollar value into the cart -- say put in $1000 worth of products instead of $25 worth of products to get a higher starting limit.  I'm not sure if there is any correlation.

depends on where you are trying to use the SCT...if its Overstock, yea, put in $500+...if its VS..$100

go to the site that you want...ex JCrew. Put like $100 worth of stuff in the shopping cart, then go check out as guest. Fill in your address as it appears on your EQ file, and you should get the pop up if you qualify. If not, clear history and browser, and try a different browser, and try again. you do not have to purchase your cart. if you get pop up, accept it, and then back out of it.

I don't think that makes much of a difference.  When I did it for Overstock I put something like a $10 item in the cart and got a CL of $12K
